Please do not fill out this preliminary registration form and mail in the $225.00 nonrefundable check unless:

1. You have been accepted into the Summer Fine Arts Program.

2. You have received a registration form and need a duplicate for more than one child in your family.

Full tuition is due on/or before May 1st.
No refunds will be issued on tuition after May 1st. unless a replacement can be made.
